I hear by release Birthday Ever After and any of
its affiliates from any claim for damage or suit
by personal injury, illness, lost, stolen or damaged
property, allergic reactions to food, costumes or
makeup to all persons at my event. I
agree to pay the remaining party cost in full on or
before the scheduled party date. I
am aware that this is a release of liability between
myself and any affiliates with Birthday Ever After
and sign of my own free will.

| Cancellations: If cancellation is made 14 days or more before your scheduled party, you will receive a full refund, minus a $25 cancellation fee. If cancellation is made 13 days or less before your scheduled party, you will forfeit your deposit. We will be happy to re-schedule on a different date upon availability. |
